Copy 3 patches from SP3 across to SP4 - they were applied shortly
after SP4 forked and I only applied them to SP3. (bnc#724947)

- patches.fixes/md-read-error-deadlock: md: fix deadlock in
  md/raid1 and md/raid10 when handling a read error (bnc#628343).
- patches.fixes/md-resync-error-deadlock: md: fix possible
  raid1/raid10 deadlock on read error during resync (bnc#628343).
- patches.fixes/nfs-report-timeout.patch: Add timeo parameter
  to /proc/mounts for nfs filesystems (bnc#616256).
