      "message": "x86/mm: Flush more aggressively in lazy TLB mode\n\nSince commit:\n\n  94b1b03b519b (\"x86/mm: Rework lazy TLB mode and TLB freshness tracking\")\n\nx86\u0027s lazy TLB mode has been all the way lazy: when running a kernel thread\n(including the idle thread), the kernel keeps using the last user mm\u0027s\npage tables without attempting to maintain user TLB coherence at all.\n\nFrom a pure semantic perspective, this is fine -- kernel threads won\u0027t\nattempt to access user pages, so having stale TLB entries doesn\u0027t matter.\n\nUnfortunately, I forgot about a subtlety.  By skipping TLB flushes,\nwe also allow any paging-structure caches that may exist on the CPU\nto become incoherent.  This means that we can have a\npaging-structure cache entry that references a freed page table, and\nthe CPU is within its rights to do a speculative page walk starting\nat the freed page table.\n\nI can imagine this causing two different problems:\n\n - A speculative page walk starting from a bogus page table could read\n   IO addresses.  I haven\u0027t seen any reports of this causing problems.\n\n - A speculative page walk that involves a bogus page table can install\n   garbage in the TLB.  Such garbage would always be at a user VA, but\n   some AMD CPUs have logic that triggers a machine check when it notices\n   these bogus entries.  I\u0027ve seen a couple reports of this.\n\nBoris further explains the failure mode:\n\n\u003e It is actually more of an optimization which assumes that paging-structure\n\u003e entries are in WB DRAM:\n\u003e\n\u003e \"TlbCacheDis: cacheable memory disable. Read-write. 0\u003dEnables\n\u003e performance optimization that assumes PML4, PDP, PDE, and PTE entries\n\u003e are in cacheable WB-DRAM; memory type checks may be bypassed, and\n\u003e addresses outside of WB-DRAM may result in undefined behavior or NB\n\u003e protocol errors. 1\u003dDisables performance optimization and allows PML4,\n\u003e PDP, PDE and PTE entries to be in any memory type. Operating systems\n\u003e that maintain page tables in memory types other than WB- DRAM must set\n\u003e TlbCacheDis to insure proper operation.\"\n\u003e\n\u003e The MCE generated is an NB protocol error to signal that\n\u003e\n\u003e \"Link: A specific coherent-only packet from a CPU was issued to an\n\u003e IO link. This may be caused by software which addresses page table\n\u003e structures in a memory type other than cacheable WB-DRAM without\n\u003e properly configuring MSRC001_0015[TlbCacheDis]. This may occur, for\n\u003e example, when page table structure addresses are above top of memory. In\n\u003e such cases, the NB will generate an MCE if it sees a mismatch between\n\u003e the memory operation generated by the core and the link type.\"\n\u003e\n\u003e I\u0027m assuming coherent-only packets don\u0027t go out on IO links, thus the\n\u003e error.\n\nTo fix this, reinstate TLB coherence in lazy mode.  With this patch\napplied, we do it in one of two ways:\n\n - If we have PCID, we simply switch back to init_mm\u0027s page tables\n   when we enter a kernel thread -- this seems to be quite cheap\n   except for the cost of serializing the CPU.\n\n - If we don\u0027t have PCID, then we set a flag and switch to init_mm\n   the first time we would otherwise need to flush the TLB.\n\nThe /sys/kernel/debug/x86/tlb_use_lazy_mode debug switch can be changed\nto override the default mode for benchmarking.\n\nIn theory, we could optimize this better by only flushing the TLB in\nlazy CPUs when a page table is freed.       "message": "mm, swap: use page-cluster as max window of VMA based swap readahead\n\nWhen the VMA based swap readahead was introduced, a new knob\n\n  /sys/kernel/mm/swap/vma_ra_max_order\n\nwas added as the max window of VMA swap readahead.  This is to make it\npossible to use different max window for VMA based readahead and\noriginal physical readahead.  But Minchan Kim pointed out that this will\ncause a regression because setting page-cluster sysctl to zero cannot\ndisable swap readahead with the change.\n\nTo fix the regression, the page-cluster sysctl is used as the max window\nof both the VMA based swap readahead and original physical swap\nreadahead.  If more fine grained control is needed in the future, more\nknobs can be added as the subordinate knobs of the page-cluster sysctl.\n\nThe vma_ra_max_order knob is deleted.       "message": "mm: page_vma_mapped: ensure pmd is loaded with READ_ONCE outside of lock\n\nLoading the pmd without holding the pmd_lock exposes us to races with\nconcurrent updaters of the page tables but, worse still, it also allows\nthe compiler to cache the pmd value in a register and reuse it later on,\neven if we\u0027ve performed a READ_ONCE in between and seen a more recent\nvalue.\n\nIn the case of page_vma_mapped_walk, this leads to the following crash\nwhen the pmd loaded for the initial pmd_trans_huge check is all zeroes\nand a subsequent valid table entry is loaded by check_pmd.       "message": "tty: fall back to N_NULL if switching to N_TTY fails during hangup\n\nWe have seen NULL-pointer dereference crashes in tty-\u003edisc_data when the\nN_TTY fallback driver failed to open during hangup.  The immediate cause\nof this open to fail has been addressed in the preceding patch to\nvmalloc(), but this code could be more robust.\n\nAs Alan pointed out in commit 8a8dabf2dd68 (\"tty: handle the case where\nwe cannot restore a line discipline\"), the N_TTY driver, historically\nthe safe fallback that could never fail, can indeed fail, but the\nsurrounding code is not prepared to handle this.  To avoid crashes he\nadded a new N_NULL driver to take N_TTY\u0027s place as the last resort.\n\nHook that fallback up to the hangup path.       "message": "Revert \"vmalloc: back off when the current task is killed\"\n\nThis reverts commits 5d17a73a2ebe (\"vmalloc: back off when the current\ntask is killed\") and 171012f56127 (\"mm: don\u0027t warn when vmalloc() fails\ndue to a fatal signal\").\n\nCommit 5d17a73a2ebe (\"vmalloc: back off when the current task is\nkilled\") made all vmalloc allocations from a signal-killed task fail.\nWe have seen crashes in the tty driver from this, where a killed task\nexiting tries to switch back to N_TTY, fails n_tty_open because of the\nvmalloc failing, and later crashes when dereferencing tty-\u003edisc_data.\n\nArguably, relying on a vmalloc() call to succeed in order to properly\nexit a task is not the most robust way of doing things.  There will be a\nfollow-up patch to the tty code to fall back to the N_NULL ldisc.\n\nBut the justification to make that vmalloc() call fail like this isn\u0027t\nconvincing, either.  The patch mentions an OOM victim exhausting the\nmemory reserves and thus deadlocking the machine.  But the OOM killer is\nonly one, improbable source of fatal signals.  It doesn\u0027t make sense to\nfail allocations preemptively with plenty of memory in most cases.\n\nThe patch doesn\u0027t mention real-life instances where vmalloc sites would\nexhaust memory, which makes it sound more like a theoretical issue to\nbegin with.       "message": "drivers: of: increase MAX_RESERVED_REGIONS to 32\n\nThere are two types of memory reservations firmware can ask the kernel\nto make in the device tree: static and dynamic.\nSee Documentation/devicetree/bindings/reserved-memory/reserved-memory.txt\n\nIf you have greater than 16 entries in /reserved-memory (as we do on\nPOWER9 systems) you would get this scary looking error message:\n [    0.000000] OF: reserved mem: not enough space all defined regions.\n\nThis is harmless if all your reservations are static (which with OPAL on\nPOWER9, they are).\n\nIt is not harmless if you have any dynamic reservations after the 16th.\n\nIn the first pass over the fdt to find reservations, the child nodes of\n/reserved-memory are added to a static array in of_reserved_mem.c so that\nmemory can be reserved in a 2nd pass. The array has 16 entries. This is why,\non my dual socket POWER9 system, I get that error 4 times with 20 static\nreservations.\n\nWe don\u0027t have a problem on ppc though, as in arch/powerpc/kernel/prom.c\nwe look at the new style /reserved-ranges property to do reservations,\nand this logic was introduced in 0962e8004e974 (well before any powernv\nsystem shipped).\n\nA Google search shows up no occurances of that exact error message, so we\u0027re\nprobably safe in that no machine that people use has memory not being reserved\nwhen it should be.\n\nThe simple fix is to bump the length of the array to 32 which \"should be\nenough for everyone(TM)\". It seems that the only reason for\nit to be strdup()-ed was a compilation warning: printk, UART and\nconsole drivers, for some reason, expect char pointer instead of\nconst char pointer. So we can just pass `of_stdout_options\u0027, but\nneed to cast it to char pointer.       "message": "powerpc/perf: Fix for core/nest imc call trace on cpuhotplug\n\nNest/core pmu units are enabled only when it is used. A reference count is\nmaintained for the events which uses the nest/core pmu units. Currently in\n*_imc_counters_release function a WARN() is used for notification of any\nunderflow of ref count.\n\nThe case where event ref count hit a negative value is, when perf session is\nstarted, followed by offlining of all cpus in a given core.\ni.e. in cpuhotplug offline path ppc_core_imc_cpu_offline() function set the\nref-\u003ecount to zero, if the current cpu which is about to offline is the last\ncpu in a given core and make an OPAL call to disable the engine in that core.\nAnd on perf session termination, perf-\u003edestroy (core_imc_counters_release) will\nfirst decrement the ref-\u003ecount for this core and based on the ref-\u003ecount value\nan opal call is made to disable the core-imc engine.\nNow, since cpuhotplug path already clears the ref-\u003ecount for core and disabled\nthe engine, perf-\u003edestroy() decrementing again at event termination make it\nnegative which in turn fires the WARN_ON.       "message": "KVM: nVMX: fix guest CR4 loading when emulating L2 to L1 exit\n\nWhen KVM emulates an exit from L2 to L1, it loads L1 CR4 into the\nguest CR4. Before this CR4 loading, the guest CR4 refers to L2\nCR4. Because these two CR4\u0027s are in different levels of guest, we\nshould vmx_set_cr4() rather than kvm_set_cr4() here. The latter, which\nis used to handle guest writes to its CR4, checks the guest change to\nCR4 and may fail if the change is invalid.\n\nThe failure may cause trouble. Consider we start\n  a L1 guest with non-zero L1 PCID in use,\n     (i.e. L1 CR4.PCIDE \u003d\u003d 1 \u0026\u0026 L1 CR3.PCID !\u003d 0)\nand\n  a L2 guest with L2 PCID disabled,\n     (i.e. L2 CR4.PCIDE \u003d\u003d 0)\nand following events may happen:\n\n1. If kvm_set_cr4() is used in load_vmcs12_host_state() to load L1 CR4\n   into guest CR4 (in VMCS01) for L2 to L1 exit, it will fail because\n   of PCID check. As a result, the guest CR4 recorded in L0 KVM (i.e.\n   vcpu-\u003earch.cr4) is left to the value of L2 CR4.\n\n2. Later, if L1 attempts to change its CR4, e.g., clearing VMXE bit,\n   kvm_set_cr4() in L0 KVM will think L1 also wants to enable PCID,\n   because the wrong L2 CR4 is used by L0 KVM as L1 CR4.       "message": "xfs: reinit btree pointer on attr tree inactivation walk\n\nxfs_attr3_root_inactive() walks the attr fork tree to invalidate the\nassociated blocks. xfs_attr3_node_inactive() recursively descends\nfrom internal blocks to leaf blocks, caching block address values\nalong the way to revisit parent blocks, locate the next entry and\ndescend down that branch of the tree.\n\nThe code that attempts to reread the parent block is unsafe because\nit assumes that the local xfs_da_node_entry pointer remains valid\nafter an xfs_trans_brelse() and re-read of the parent buffer. Under\nheavy memory pressure, it is possible that the buffer has been\nreclaimed and reallocated by the time the parent block is reread.\nThis means that \u0027btree\u0027 can point to an invalid memory address, lead\nto a random/garbage value for child_fsb and cause the subsequent\nread of the attr fork to go off the rails and return a NULL buffer\nfor an attr fork offset that is most likely not allocated.\n\nNote that this problem can be manufactured by setting\nXFS_ATTR_BTREE_REF to 0 to prevent LRU caching of attr buffers,\ncreating a file with a multi-level attr fork and removing it to\ntrigger inactivation.\n\nTo address this problem, reinit the node/btree pointers to the\nparent buffer after it has been re-read.       "message": "xfs: Don\u0027t log uninitialised fields in inode structures\n\nPrevent kmemcheck from throwing warnings about reading uninitialised\nmemory when formatting inodes into the incore log buffer. There are\nseveral issues here - we don\u0027t always log all the fields in the\ninode log format item, and we never log the inode the\ndi_next_unlinked field.\n\nIn the case of the inode log format item, this is exacerbated\nby the old xfs_inode_log_format structure padding issue. Hence make\nthe padded, 64 bit aligned version of the structure the one we always\nuse for formatting the log and get rid of the 64 bit variant.       "message": "media: dvb_frontend: only use kref after initialized\n\nAs reported by Laurent, when a DVB frontend need to register\ntwo drivers (e. g. a tuner and a demod), if the second driver\nfails to register (for example because it was not compiled),\nthe error handling logic frees the frontend by calling\ndvb_frontend_detach(). That used to work fine, but changeset\n1f862a68df24 (\"[media] dvb_frontend: move kref to struct dvb_frontend\")\nadded a kref at struct dvb_frontend. So, now, instead of just\nfreeing the data, the error handling do a kref_put().\n\nThat works fine only after dvb_register_frontend() succeeds.\n\nWhile it would be possible to add a helper function that\nwould be initializing earlier the kref, that would require\nchanging every single DVB frontend on non-trivial ways, and\nwould make frontends different than other drivers.\n\nSo, instead of doing that, let\u0027s focus on the real issue:\nonly call kref_put() after kref_init(). That\u0027s easy to\ncheck, as, when the dvb frontend is successfuly registered,\nit will allocate its own private struct. So, if such\nstruct is allocated, it means that it is safe to use\nkref_put().
